Do high-protein diets actually carry health risks? Fact-checking the claim making the rounds
A "high protein diets are as dangerous as smoking" style claim keeps resurfacing online, usually tracing back to one specific study. The study is real. The way it's being summarised isn't the full picture, and the much larger body of evidence since tells a different story.
What the research actually shows
The source is almost always a 2014 Cell Metabolism paper that followed 6,381 US adults aged 50 and over for 18 years using NHANES survey data. In the 50-65 age group, people eating a high-protein diet had over four times the risk of cancer death compared to those eating low protein, a hazard ratio of 4.33 (Levine et al., 2014). That number is genuinely striking, and it's the number driving most of the viral claims. What usually gets left out: in adults over 65, the exact same study found the relationship reversed, higher protein intake was associated with lower all-cause and cancer mortality. And this is an observational study built on a single 24-hour dietary recall, not a controlled trial, it can show an association, not prove protein caused the deaths. Independent experts reviewing the paper at the time flagged that a 4-fold risk increase from diet alone is a larger effect than almost anything seen in nutritional epidemiology, and noted the analysis didn't fully account for confounders like smoking, obesity, and overall diet quality.
The much larger, more recent picture: a 2020 dose-response meta-analysis in the BMJ, pooling 32 cohort studies and 715,128 people with over 113,000 deaths recorded, found higher total protein intake was associated with slightly lower all-cause mortality, not higher (Naghshi et al., 2020). Plant protein specifically was linked to meaningfully lower mortality risk; animal protein showed no significant association with cardiovascular or cancer mortality either way. This doesn't erase the 2014 finding in the 50-65 subgroup, but it means the "high protein diets carry major health risks" headline isn't supported by the weight of the broader evidence.
On the specific fear people usually mean by "risk," kidney damage: a 2018 meta-analysis in the Journal of Nutrition, pooling 28 randomised controlled trials and 1,358 participants without pre-existing kidney disease, found no adverse effect on kidney function from higher-protein diets when comparing actual changes in glomerular filtration rate from baseline (Devries et al., 2018). In people with healthy kidneys to start with, this specific fear isn't backed by the trial evidence.
What this means for you
If you're a healthy adult under 65 without kidney disease, the current evidence doesn't support "high protein is dangerous" as a blanket claim. The one genuine, specific signal worth taking seriously is the IGF-1 mechanism in the 50-65 group from the original study, IGF-1 is a growth-promoting hormone that some cancer biology research links to tumour growth at chronically elevated levels. If you're in that specific age bracket and eating very high protein long-term, particularly from processed sources, that's a reasonable thing to be aware of, not a reason to fear normal high-protein eating broadly.
The biotype angle
Ectomorphs and mesomorphs actively building or maintaining muscle typically need protein intake in a range this controversy doesn't meaningfully touch, the concerning NHANES threshold for "high protein" was over 20% of calories, which is a moderate intake by most training-focused standards, not an extreme one. Anyone over 65, especially those at risk of sarcopenia, should if anything be paying more attention to adequate protein, not less, given both the reversed mortality signal in the original study and the well-established muscle-preservation evidence for older adults.
The honest verdict
The viral version of this claim overstates what one heavily-caveated observational study in a specific age bracket actually found, and ignores that the same study reversed for people over 65. The larger, more recent meta-analytic evidence doesn't support a general health risk from high protein intake in healthy adults, and the kidney-damage fear specifically isn't backed by RCT data in people without existing kidney disease. The genuine nuance worth keeping: this isn't "protein is risk-free at any level for everyone," it's that the specific "major health risk" framing circulating right now is doing more work than the underlying research supports.
